What personal information I collect

When you make an enquiry through my website contact function (via drgemmaparry.com), I collect personal details including your name, email address and phone number (if added). This information is used to contact you about your query. If you make an appointment to see me I collect the following details as minimum - name, address, email, phone number and GP address. This information will be kept electronically in a password protected document. Hard copies of this information will be shredded and disposed of.

Whilst you use my website, I also receive information about your computer such as your IP address, operating system and browser details. This information helps me provide a better website experience for you.

How and why I collect your personal information

For professional and legal reasons, if I work therapeutically with you I am required to keep your personal information and brief details of our sessions (case notes) for a period of 7 years after our final appointment. After this 7 year period, I will permanently delete your information and case notes.

If you contact me and decide not to undertake psychological therapy, I will keep your personal contact information (name, telephone number, email address) for a period of 6 months, after which I will delete it.

How I use your personal information

I use your demographic information to contact you as required. I keep brief session notes with the intention of best practice, for example to read notes from previous sessions prior to next sessions and to complete documentation such as therapy summary letters where this is requested. This information is kept virtually in a password protected document on an encrypted cloud server, marked by only your initials. Your contact details are stored separately to session notes.

I am required to share information that you tell me without your consent only if I am liable to as part of civil or criminal court proceedings, or if you indicate an imminent risk of loss of life to you or another person. In these cases only information which is relevant to the situation would be shared, and where practicable I would attempt to discuss information sharing with you before proceeding.

How I secure your personal information

All electronic information is stored in password protected documents on an end-to-end encrypted cloud server which complies with guidelines in the General Data Protection Regulation (GDPR).

How you can access, update or amend your personal information

You have the right to view the personal information and records I keep about you and your therapy sessions. You also have the right to request I update or amend your data if it is incorrect. Requests to view your information/records or make any amendments can be made to me by email.

How you can make a complaint about how I have handled your personal information

You have the right to file a complaint to the Information Commissioners Office (ICO) if you are unhappy with how I have managed your personal information.
